Phó Thủ tướng Vũ Đức Đam nhấn mạnh điều này tại Hội nghị của Chính phủ với các địa phương triển khai Nghị quyết của Quốc hội về thực hiện các giải pháp cấp bách phòng chống dịch.
Phát biểu tại Hội nghị, Phó Thủ tướng Vũ Đức Đam nhấn mạnh, hệ thống y tế của Việt Nam nói chung, đặc biệt là hệ thống điều trị, không thể so với các nước phát triển. Không có đại dịch Covid-19 thì các bệnh viện vẫn quá tải, nhất là các bệnh viện tuyến cuối. Vì thế, chúng ta vẫn phải kiên định chiến lược chống dịch của một nước đang phát triển, dựa vào đặc thù riêng của hệ thống chính trị của Việt Nam, dưới sự lãnh đạo của Đảng và truyền thống của nhân dân.
Không được nhầm những biện pháp điều chỉnh cho khu vực TP. HCM mà áp dụng cho cả nước
Theo Phó Thủ tướng, chúng ta đã kiểm soát tốt dịch bệnh trong 3 lần trước. Đợt dịch lần thứ tư có chủng mới lây lan nhanh, đã lây vào khu công nghiệp, bệnh viện lớn. Nhưng ngay trong đợt dịch thứ tư, chúng ta đã kiểm soát thành công cho đến khi dịch bùng phát tại TP. HCM và một số địa bàn giáp ranh TPHCM như các tỉnh Bình Dương, Đồng Nai, Long An (khu vực TP. HCM).
Khu vực này có đặc điểm rất đông dân cư, đặc biệt là công nhân khu công nghiệp với mật độ rất cao, lây lan rất nhanh. Vì vậy, chúng ta phải có cách tiếp cận, giải pháp đặc biệt dành riêng cho khu vực này.
Phó Thủ tướng cho biết suốt những ngày qua, ông đã trực tiếp tìm hiểu, trao đổi với lãnh đạo TP. HCM, bộ phận thường trực của các bộ, ngành, báo cáo với Thủ tướng, thống nhất một số điều chỉnh: “Chúng ta không được nhầm những biện pháp điều chỉnh cho khu vực TP. HCM mà áp dụng cho cả nước thì rất nguy hiểm. Ngay trong khu vực TPHCM, có những vùng còn tương đối an toàn vẫn phải thực hiện truy vết đến F1, F2, ví dụ như một số xã ở huyện Củ Chi, huyện Cần Giờ “.
Phó Thủ tướng cho rằng mặc dù năng lực xét nghiệm của các địa phương tăng lên nhanh, với nhiều đổi mới cụ thể nhưng một số nguyên tắc không được thay đổi. Đầu tiên là phải áp dụng các biện pháp chống dịch sớm hơn một bước, cao hơn một mức.
“Chúng ta phải áp dụng các biện pháp sớm hơn, cao hơn, nhất định không được muộn hơn, thấp hơn”, Phó Thủ tướng nêu rõ và lấy ví dụ trong 19 tỉnh, thành phố phía nam đang thực hiện Chỉ thị 16 thì các tỉnh phía nam sông Hậu và Bình Phước chưa đến mức phải làm như vậy nhưng vẫn phải thực hiện. Hoặc đối với TP. Hà Nội, khi hệ thống cảnh báo mức đỏ, Phó Thủ tướng đã trực tiếp trao đổi với lãnh đạo thành phố để báo cáo Thủ tướng Chính phủ áp dụng Chỉ thị 16.
Đồng thời, theo Phó Thủ tướng, chúng ta vẫn phải tiếp tục kiên trì chiến lược ngăn chặn, phát hiện, cách ly, khoanh vùng, dập dịch và điều trị. Cụ thể, vẫn phải truy vết, cách ly F0, F1 trừ những nơi chúng ta thí điểm dần dần cách ly F0 ở nhà an toàn, còn những nơi có ít ca nhiễm thì phải truy vết đến cùng, thậm chí là F2, F3.
Khoanh vùng, thực hiện giãn cách phải nghiêm ngay từ đầu. Chỉ cần chúng ta thực hiện đúng theo Chỉ thị 16, ai ở đâu yên đấy, nhà cách ly với nhà thì dịch bệnh sẽ không bùng phát.
Các địa phương quán triệt tinh thần khoanh hẹp nhỏ nhất có thể, còn khi chưa đủ căn cứ thì tạm thời khoanh rộng, khẩn trương điều tra dịch tễ để khoanh gọn lại.
Tiếp theo là công tác xét nghiệm phải có trọng tâm, trọng điểm, bảo đảm hiệu quả và tiết kiệm. Phó Thủ tướng nhắc nhở: “Dàn hàng ngang xét nghiệm, ngoài việc tốn kém chi phí thì còn rất nguy hiểm vì gây tâm lý chủ quan cho người dân, khi kết quả xét nghiệm nhanh âm tính thì không tuân thủ đầy đủ 5K”.
Mặt khác, do dịch đã nhiễm rất rộng, ngấm rất sâu ở khu vực TP. HCM nên Phó Thủ tướng Vũ Đức Đam cho rằng cần có những biện pháp đặc biệt.
Thứ nhất, khu vực này phải thực hiện giãn cách xã hội nghiêm ngặt để giảm lây lan.
Thứ hai, cả chính quyền đoàn thể và các tổ chức làm thiện nguyện cùng tham gia chăm lo đời sống hằng ngày cho người dân.
Thứ ba, phải xây dựng hệ thống giám sát y tế tại cộng đồng bảo đảm bất kỳ ai trong khu vực dân cư nào có triệu chứng nhiễm Covid-19 hoặc các bệnh khác cần có sự hỗ trợ y tế thì phải được tiếp cận và hỗ trợ ngay. Có như vậy, người dân mới yên tâm ở trong nhà.
Đối với điều trị, Phó Thủ tướng Vũ Đức Đam nhấn mạnh phải rất sáng tạo và thực tiễn. Từ mô hình điều trị “tháp 3 tầng” của Bộ Y tế, TP. HCM đã phân chia thành 5 lớp: lớp đầu tiên tiếp nhận người nhiễm Covid-19 không triệu chứng cần chăm lo, quan tâm đầy đủ cho bà con từ chế độ ăn uống, vận động đến hỗ trợ nhau trong sinh hoạt hằng ngày. Lớp thứ nhất, thứ hai thì chính quyền cơ sở lo được, còn ngành y tế thì chuyên sâu vào lớp thứ tư, thứ năm và một phần lớp thứ ba. Mỗi lớp điều trị đều đặt chỉ tiêu để giảm tỉ lệ số ca phải chuyển lên tuyến trên.
Tinh thần cả nước hướng về TP. HCM
Phó Thủ tướng Vũ Đức Đam cũng cho biết, Chính phủ giao Bộ Y tế phân bổ vaccine phòng Covid-19 cho các địa phương nhưng trong lúc này, tình hình tại khu vực TPHCM đã khác.
Chiến lược hiệu quả nhất lúc này đối với khu vực TP. HCM là phải tập trung tiêm vaccine hết cho người dân trong thời gian sớm nhất có thể để tạo miễn dịch cộng đồng. Sau đó, sẽ khoanh gọn khu vực này, không để dịch lây lan ra khu vực khác. Điều này rất quan trọng bởi vì vaccine chỉ có đầy đủ tác dụng bảo vệ sau khoảng 1,5 tháng.
Người tiêm vaccine rồi vẫn có thể bị nhiễm và lây cho người khác nhưng tỷ lệ diễn biến bệnh nặng sẽ ít đi. Nếu không, số người bị bệnh nặng và tử vong sẽ rất nhiều, khi toàn bộ hệ thống điều trị, dù có tăng cường chi viện thêm, nhưng sau thời gian dài sẽ vô cùng mệt mỏi, để lây nhiễm trong đội ngũ y tế.
Hiện nay, lô vaccine nào về Bộ Y tế cũng đứng trước sức ép ký phân bổ ngay cho các địa phương, dù có ưu tiên cho Hà Nội, TP. HCM nhưng sẽ không đủ thực hiện chiến lược có miễn dịch cộng đồng sớm nhất cho khu vực TP. HCM.
Phó Thủ tướng nói: “Tôi không biết nói gì hơn và tha thiết đề nghị Bộ Y tế, các tỉnh, thành phố trong thời điểm này, nhường một phần vaccine để TP. HCM tiêm trước cho người dân nhằm đạt được miễn dịch cộng đồng. Đây là tinh thần cả nước hướng về TP. HCM. Thời điểm này từng liều vaccine cực kỳ quý giá sẽ giúp được TP. HCM chống dịch và cứu được nhiều người hơn”.
Phó Thủ tướng cho rằng nếu đạt được sự thống nhất như vậy, TP. HCM sẽ tuyên truyền cho người lao động ngoại tỉnh ở lại tiêm vaccine. Ông đã bàn với lãnh đạo thành phố, sau khi tiêm xong vaccine sẽ tổ chức cho công nhân đi làm tiếp, hoặc làm các công việc công ích vừa có thu nhập, giảm bớt cứu trợ, vừa giúp mọi người được lao động trong trạng thái bình thường mới.
Mặc dù quy định, kế hoạch về tiêm vaccine của Bộ Y tế rất tốt, khoa học nhưng với điều kiện hiện nay, Phó Thủ tướng Vũ Đức Đam đề nghị giao cho TP. HCM lập kế hoạch tiêm vaccine nhanh nhất có thể. Ông sẽ bàn rất kỹ với lãnh đạo TP. HCM về phương án tiêm vaccine theo cụm thay vì theo đối tượng. Nếu lượng vaccine hiện tại chưa tiêm hết thì những lô vaccine tới đây sẽ tập trung cho TP. HCM. Còn chung cả nước, chúng ta phải vẫn phải thực hiện nghiêm các văn bản phòng, chống dịch từ trước đến nay, cố gắng khống chế, không để dịch bệnh lan như TP. HCM và một số nơi.
